My Plate Survey

Introduction

UMass Dining is conducting a health promotion for the 2011-2012 academic year. Please fill out the information below and fill out the survey each week to be eligible for the gift cards (details below). We ask you to complete the survey the same day every week to maintain consistency. Thank you for participating in the survey! Have a great semester!

Terms and Conditions:

  • Participants must be on a meal plan to be eligible for prizes.
    Surveys begin at the beginning of each semester. Students will record the amount of milk (or milk alternatives), yogurt, whole grains, and refined grains consumed, as well as the percentage of fruits and vegetables at one dinner meal each week.
  • To maintain consistency, please submit the survey the same day every week.
  • Students can only fill out the survey for that week until Saturday at 11:59 p.m.
  • Students are allowing UMass Dining to utilize group (not individual) data from this promotion for contest entries and disclosing results at conferences.
  • Each student who submits a survey on time in one week will be eligible for a drawing for a $50 gift card. If a student submits more than one survey per week, their name will only be entered one time.
  • Each student who submits a survey EVERY week from January 23 to April 28 is eligible for one of five $200 gift cards.
  • Students who come to nutrition promotions in the dining commons during the semester will get a chance for a second entry for the drawing that week. Details will be e-mailed and put on Facebook.
